Sample Disability Statements for Course Syllabi
Accessibility Services strongly suggests that all course instructors include a statement on every course syllabus acknowledging the unique learning needs of students and outlining the process of how students with disabilities may request academic accommodations.
Sample statements available for use on a course syllabi:
The college makes reasonable accommodations for persons with disabilities. Students should notify Accessibility Services at the Center for Student Success in Peters Hall rooms 116/118 and their instructor of any disability related needs, including any academic adjustments or accommodations requested.
If you have a physical, psychiatric, or learning disability and require accommodations, please let me know early in the semester so that your learning needs may be appropriately met. You also will need to meet with Accessibility Services in Peters Hall rooms 116/118.
If you have a disability that may impact your work in this class and you may require accommodations, please see me. You should also visit Accessibility Services at the Center for Student Success so that we may arrange appropriate accommodations.

Accessibility Services staff also suggest that course instructors clearly identify the essential elements or fundamental components of each course. These may be included in statements concerning a student’s attendance or participation requirements, intended learning outcomes, and established due dates or deadlines for exams and assignments.
This information is important to each and every student; however, it is crucial when determining the reasonableness of requested accommodations for students with disabilities.
