GROUP Project Forms (to be completed by Sponsor)

Winter Term 2010 Forms

Group Project Grant Application - To apply for a grant to help fund a group project.

Group Project Grant Budget Proposal - To be included with either the Early-funded Grant Application or the Group Project Grant Application.

Group Project Publicity Form - To advertise a group project on the Winter Term website.  If you apply for a group grant, you must also include this form.

International Group Winter Term Project Forms

Sponsor Number Assignment Form for A&PS - In order to sponsor a Winter Term project, members of the A&PS staff must complete this form and submit to the Registrar's Office.

Final Report Forms

• Financial report of actual expenses (must be submitted on the same Budget Proposal form used at the time of application for the grant). Attach ORIGINAL receipts only; copies not acceptable.

  Final Report must be returned to the Winter Term Office.

INDIVIDUAL Project Forms (to be completed by Student)

On-Campus Project Proposal -- Student must submit this form to his/her sponsor for an individual on-campus project.  If applying for an Individual Project Grant, you must also include this form.

Off-Campus Project Proposal -- Student must submit this form to his/her sponsor for an individual off-campus project.  If applying for an Individual Project Grant, you must also include this form.

Individual Project Grant Application --To apply for money for an individual project. (Note: an individual grant is a reimbursement and is not paid in advance.)

Registration Card -- Must be completed by student and signed by the project sponsor and returned to the Office of the Registrar by the registration deadline December 2, 2009.  Extra cards are available from the Office of the Registrar or The Winter Term Office. This form is not available online.

A student proposing an individual project must submit one of the Project Proposal forms to their sponsor along with the Registration Card.
