Oberlin Academic Leave of Absence Checklist

1) Completed Application
2) Completed online Academic Leave Approval Form
3) Preliminary Transfer of Credit Forms (available to download from this page or the online
application)
*Preliminary approval is not required for elective courses taken on affiliated programs. 

*Courses taken that you intend to count toward major or minor requirements must have preliminary approval. 
*Any course taken on a non-affiliated program must have preliminary approval.
* If you are applying to more than one program, you must include Preliminary Transfer of Credit Forms for each program and write about each program in your essay.
4) Proof of International Student Identification Card
*Submit a copy of your International Student ID Card or letter of proof that your
program provides the card.
