Summary: This tool allows for data from a form to be stored in a database and retrieved whenever needed in a format that will open easily in Microsoft Excel.

To make this work, your form must use a particular action and at least 2 hidden fields. There are also a number of dos and don'ts. For detailed instructions on how to create your form, click here.

Once your form has been made available to the public, the data can be retrieved by logging in here.
