Who
is Eligible for coverage?
All active
employees working a minimum of 30 hours each week can purchase coverage.
You have the option to purchase AD&D coverage for yourself regardless
of whether you purchase Term Life coverage. You may also purchase
Term Life coverage for your spouse/partner and your children between
the ages of live birth and 19, 26 if the child is a full-time student.
AD&D coverage is available for your spouse/partner and your
children up to the age of 19, 26 if the child is a full-time student.
In order to purchase Term Life and AD&D coverage for your spouse/partner
and children, you must enroll in Term Life and AD&D coverage
for yourself. The Beneficiary
Card is available on line.
How
much coverage is available?
Employee
Term Life coverage is available in $10,000 increments up
to 5 times your salary to a maximum of $500,000.
Employee
AD&D coverage is available in $10,000 increments up
to 5 times your salary to a maximum of $500,000. Employees age 70
or older should ask their plan administrator for the amount of coverage
available to them.
Spouse
Term Life coverage is available in $5,000 increments to
a maximum of $250,000.
Spouse
AD&D coverage is available in $5,000 increments to a
maxiumum of $250,000.
Dependent
Child Term Life coverage is available in $2,000 increments
to a maximum of $10,000 for each child. The maximum death benefit
for a child between the ages of live birth to 6 months would be
$100.
Dependent
Child AD&D coverage is available in $2,000 increments
to a maximum of $10,000 for each child.
NOTE: The
maximum amount of Term Life and AD&D coverage you may purchase
for your spouse and children is 50 percent of the amount you choose
for yourself (within the coverage maximums stated above). Please
see the Department of Human Resources if you have any questions.
