Dental
Plan
An employee
may only enroll for Dental coverage when they are first eligible
or during the annual Open Enrollment period. During the Open Enrollment
period, you may enroll or cancel the Dental plan. Employees who
are currently enrolled and wish to continue their current coverage
need to do nothing during Open Enrollment.
There are
three dental plans:
(1)(2) Superior Dental Care: New in 2011 is the offering of two dental plans from Superior Dental Care. The Core plan provides coverage for basic services such as cleaning, fluoride treatments for covered children up to age 19, x-rays, and exams. The Enhanced plan will provide all of the services offered with the Core plan, but has coverage for major services such as oral surgery, prosthodontics, and crowns. The Enhanced plan also has coverage for orthodontic services for children under age 20.

(3) CIGNA
HMO Dental Plan: The CIGNA HMO plan provides coverage on a fee basis
schedule for basic services and major dental services such as crowns,
dentures, and orthodontics (adult and child). However, just as with
any HMO plan, coverage is limited to network providers. To check
CIGNA HMO Dental Plan providers, go to www.cigna.com,
then provider directory, choose dental and then put in your zip
code. Also, consult the patient
charge schedule. At the next screen, choose HMO. When choosing
a dental plan, make sure you review the list of network providers,
and understand the types of covered services, and the amount of
out-of-pocket expenses you can expect.
