OBERLIN COLLEGE PRIVACY POLICIES AND PROCEDURES

DEFINITIONS


The following are definitions applicable to the implementation of the HIPAA Privacy Policy and Procedures.

Protected Health Information ("PHI") is generally information that:

Is created or received by a Covered Entity or Employer
Relates to past, present, or future physical or mental health condition and provision of or payment for health care and
Permits individual identification
May be in any form - oral, written or electronic

Business Associates are entities that perform functions for or provide services to or on behalf of a Covered Entity when the function or service involves the use or disclosure of individually identifiable health information, including claims processing or administration. Examples of these are Third Party Administrators, Prescription Benefit Administrators, Flexible Spending Account Administrators, Utilization Review and Case Managers and Disease Management Administrators, consulting firms, law firms, accounting firms, and audit firms.

Covered Entity means the Plan, not the College.
